Your loved one will be well cared for and safe in his or her own home with a qualified caregiver. Or you can opt for your loved one to stay overnight in a memory care facility. 

To qualify for periodic respite care services, the client must

  1. Live in Smith County or agree to receive services in Smith County
  2. Have a medical diagnosis of Alzheimer’s or another form of dementia from his or her medical provider
  3. Complete the REACH program.
  4. Have a consultation with Alzheimer’s Alliance Respite Coordinator
  5. Provide proof of household income to qualify for services.
  6. Agree that the Alzheimer’s Alliance will be a payer of last resort.
